A new physical exam is required for ALL student-athletes PRIOR to the start of the new school year. Students are required to use the Physical Form link provided above.

No student will be allowed to participate in a sport until their PHYSICAL EXAM FORM has been turned into the Athletic Department AND all Electronic Athletic Participation Forms have been submitted online and approved by the Athletic Department.
